1. Person in charge present, demonstrates knowledge, and performs duties
- comments: pic on site has no city of chicago food sanitation certificate. Instructed to obtain and maintain a valid certificate. Priority foundation citation written on violation #2.

2. City of chicago food service sanitation certificate
- comments: observed the person in charge without an original/valid city of chicago food service sanitation managers certificate. Instructed the person in charge that is on site must always have an original/valid city of chicago food service sanitation managers certificate posted. Priority foundation. Citation issued 7-38-012.

38. Insects, rodents, & animals not present
- comments: observed evidence of insect activity on site. Observed about 60 live small flies on the walls/ceiling, storage racks and conduits in the kitchen/dish washing area. Instructed to eliminate small fly activity, clean and sanitize affected areas, and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).

41. Wiping cloths: properly used & stored
- comments: observed wiping clothes improperly stored, on handsinks, prep tables, instructed to store in properly in sanitizing solution (must be at the required ppm) when not in use. Maintain same.

49. Non-food/food contact surfaces clean
- comments: storage racks in the food prep areas, dish washing area and inside the walk in cooler with dirt/dust and food. Instructed to clean and maintain storage racks. -must clean and maintain the lower surface of all of the coolers and freezers to remove food and dirt debris.

51. Plumbing installed; proper backflow devices
- comments: the base of the faucet on the 3-compartment sink leaks when turned on. Instructed to fix and maintain good plumbing.

55. Physical facilities installed, maintained & clean
- comments: floor inside the walk in cooler is in poor repair and is a trip hazard. Instructed to fix floor and maintain a smooth and easily cleanable surface.

- comments: floors in the corners, under/around all heavy equipment, ice machine, inside the walk in cooler/freezer with food and dirt build up. Instructed to clean and maintain floors. -walls and ceiling tiles in the food prep area around all sinks, cooking/cooling equipment and dish machine with food splatter, dirt and dust build up. Instructed to detail clean and maintain walls and ceiling tiles throughout.

55. Physical facilities installed, maintained & clean
- comments: mops not properly stored when not in use. Instructed to store mops hanging up when not in use for proper air drying. Maintain same.

60. Previous core violation corrected
- comments: previous core violations from report #2612943 dated 02/27/2025 not corrected and/or maintained. Instructed to correct and maintain: -#49. 4-601. 11(c) observed accumulated grease debris between the fryers. Instructed to clean. -#55. 6-501. 12 observed excessive grease and food debris under the fryers in the food prep area. Instructed to clean and sanitize. -#58. 2-102. 13 observed no allergen training certificate was provided for the inspector's review. Instructed to meet requirements for all food service sanitation managers. Priority foundation 7-42-090 citation issued
