- comments: observed 2 ice machines located near the dishwash area with a build-up of black and pink slime substance with ice cubes stored underneath in ice bin. Ice is used for human consumption. Instructed to clean, sanitize and maintain. Priority foundation violation. 7-38-005. Citation issued.

- comments: observed ice scooper being stored improperly on the top of the ice bin. Instructed on how to properly store ice scoop between usage.

- comments: observed no ambient air thermometer inside the reach in prep area cooler(s). Instructed to provide inside all refrigerator units to properly monitor the ambient air temperature of equipment.

- comments: observed food storage containers without labels containing the common name of foods. Instructed to provide.

- comments: observed food containers stored on the floor in the walk-in cooler. Instructed to store properly by elevating at least six inches from the floor on raised shelving units.

- comments: observed scoops for dry storage containers being stored directly inside of the container. Instructed to store properly between use.

- comments: observed rusted surfaces on storage racks/shelving units in walk in cooler and storage areas. Instructed surfaces must be smooth and easily cleanable.

- comments: observed no temperature measuring device for warewashing machine. Observed broken/inoperable warewashing unit temperature measuring gauges (wash, rinse and sanitize). Instructed to correct/repair. Priority foundation violation. 7-38-005. Citation issued.

- comments: observed an excessive amount of debris and ice buildup on the floor/walls/ceiling of the walk in freezer. Instructed to remove excess ice, clean and maintain.

- comments: observed outside dumpster with missing/broken lids. Instructed to correct, replace and maintain to ensure tight fitting lids. Priority foundation violation. 7-38-020(b). Citation issued.

- comments: observed floors with excess water, grease and debris in the dishwash area and near the dish machine area. Instructed to detail clean.

- comments: observed the ventilation hood and filters above the cooking equipment in need of cleaning. Observed overhead vents thorughout the facility with excessive dirt/debris and dust. Instructed to detail clean and maintain.
