37. Food properly labeled; original container
- comments: 3-302. 12: observed bulk food containers in rear storage area not labeled with the common name of the stored food. Instructed to label all bulk food with common name of stored food and maintain.

39. Contamination prevented during food preparation, storage & display
- comments: 3-307. 11: observed cup used as scoop in bulk dry storage container. Instructed to remove cup and replace with utensil with a handle and maintain.

43. In-use utensils: properly stored
- comments: 3-304. 12: observed in-use scoop stored in still water next to customer self-service rice cooker. Instructed to remove scoop from water and store on dry, clean surface and maintain.

44. Utensils, equipment & linens: properly stored, dried, & handled
- comments: 4-904. 11: observed consumer self-service utensils stored with lip-contact surfaces facing upwards. Instructed to invert utensils for self-service so that lip-contact surfaces are protected from contamination and maintain.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: 4-202. 15: observed can opener in kitchen area rusted and damaged beyond repair. Instructed to replace can opener and maintain in good repair.

- comments: 4-601. 11(b): observed the following food equipment and contact surfaces not clean: interior/exterior of the deep fryers/cooking equipment with accumulated food, grease and dirt build up. Storage racks with food and dirt debris in the food prep area and inside the walk in coolers. Lower surface of prep tables -rubber gaskets on the walk/reach in coolers. Instructed to detail clean and maintain all.

49. Non-food/food contact surfaces clean
- comments: 4-601. 11(c): observed faucets of all handwashing sinks and exterior of all bulk food storage containers soiled with debris. Instructed to clean all surfaces of equipment at adequate frequency to prevent build up of debris and maintain.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: 5-501. 17: observed toilet room receptacles (women) left uncovered. Instructed to provide covers for all waste receptacles in toilet rooms at all times and maintain.

55. Physical facilities installed, maintained & clean
- comments: 6-501. 12: observed floors with excess water, dirt, food, grease and debris through out including the walk in coolers/freezers. Instructed to detail clean and maintain floors through out. Walls and ceiling with food, dirt and grease build up in all prep and storage areas, around sinks and heavy equipment, and employee restroom. Instructed to detail clean and maintain all.
