High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee manipulates coleslaw without gloves, later put gloves but no hand wash.

High Priority - Food preparation has soil/old food residue. Observed inside walk in cooler food trays soiled.

High Priority - Live, small fly insects at food storage area. Observed approximately 30 plus flies coming out and around the drain in that room.

High Priority - Roach activity present as evidenced by live roaches found. Observed approximately 3 live roaches by reach in cooler at front counter by sushi station. Observed also at second floor approximately three live roaches by two compartment sink. Observed one live roach on floor by the dessert unit at main seating area.

High Priority - Roach excrement and/or droppings present. At kitchen above hand sink located in the corner by the fryers.

High Priority - Rodent activity present as evidenced by rodent droppings found. Observed approximately 10 rodent droppings by prep sink located at second floor.

High Priority - Stop Sale issued on time/temperature control for safety food due to temperature abuse. Chicken at 63°F, cut lettuce 54°F.

High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed at reach in cooler at cook line cooked chicken at 63°F, cut lettuce 54°F.
