- comments: 6-301. 14 observed hand washing signs are needed at all the hand washing sinks in all food prep areas. Instructed to provide.

39. Contamination prevented during food preparation, storage & display
- comments: 3-305. 11 observed facility storing containers of ice, food and salsa on the ledge in between the food prep area and customer dining area without lids. Instructed to provide lids for all or store all foods behind the food prep area counter.

45. Single-use/single-service articles: properly stored & used
- comments: 4-502. 13 observed cardboard being reused as a floor liner in the dish washing area. Instructed to not reuse cardboard, all surfaces must be smooth and easily cleanable.

- comments: 5-501. 17 observed no covered receptacle in the employee toilet room located in the basement prep area. Instructed to provide.

- comments: 2-102. 13 observed manager unable to locate the food handlers certificates for the male food handlers. Instructed to provide.

- comments: 2-102. 13 observed the city of chicago food service sanitation manager without the required allergen training certificate. Instructed to meet requirement.

- comments: observed facility unable to provide a written procedure for implementing and enforcing vaccination status for in door dining customers. Instructed to provide. No citation issued.
