- comments: 2-101. 11 observed the person in charge without a valid city of chicago food service sanitation manager's certificate. Instructed to obtain a valid city of chicago food service sanitation manager's certificate. Priority foundation. Citation issued 7-38-012.
5. Procedures for responding to vomiting and diarrheal events
- comments: 2-501. 11 observed no written procedure nor adequate supplies on site for responding to vomit and diarrhea events that can potentially occur inside of the facility. Instructed to provide at all times. Priority foundation. Citation issued 7-38-005.
9. No bare hand contact with rte food or a pre-approved alternative procedure properly allowed
- comments: 3-301. 11(b) observed bare hand contact with ready-to-eat foods. Observed a male food handler chopping and handling ready-to-eat lettuce with his bare hands during the inspection. Instructed to discard said rte foods, always wash hands, and apply food-grade disposable gloves to handle all ready-to-eat foods. Employee discarded said rte food, washed hands, and applied food-grade disposable gloves. Priority violation. Citation issued 7-38-010.
16. Food-contact surfaces: cleaned & sanitized
- comments: 4-602. 11(b-e) observed the interior of the ice machine with a brownish substance located by the ice maker. Instructed to clean and sanitize.
38. Insects, rodents, & animals not present
- comments: 6-501. 111 observed evidence of pest activity on site. Observed about 5 live large flies flying around the rear food prep area and about 10 live small flies on the walls in the toilet room. Instructed to eliminate live insect activity, clean and sanitize affected areas, and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).
45. Single-use/single-service articles: properly stored & used
- comments: 4-502. 13 observed cardboard being reused as a shelf liner on the lower shelf of prep tables in the food prep area. Instructed not to reuse cardboard. All surfaces must be smooth and easily cleanable.
46. Gloves used properly
- comments: 3-304. 15 observed food grade disposable gloves not being used the handle ready to eat foods. Instructed to review when to use food grade disposable gloves with all food handlers.
51. Plumbing installed; proper backflow devices
- comments: 5-204. 12 unable to locate the back flow prevention device on the ice machine. Instructed to locate or provide.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: 6-501. 19 observed no self closing device on the toilet room door that leads to the food prep area. Instructed to provide a self closing device.

- comments: 6-501. 12 observed floors need to be cleaned in the food prep area under the 3 compartment sink. Instructed to clean.
