- comments: observed food handler chewing gum while preparing food (chopping onions). Instucted to eatin deignated areas away from exposed food to prevent possible contamination.

- comments: observed approximatley 65 small live black lfies flying around dishwashing room, prep area, dry storage on walls and storage carts, hallway leading to walk in coolers and exit doors on walls and on exterior surfaces of cooling units. Instructed to contact pest control for additional service and eliminate pest activity. Priority foundation violation. #7-38-020 (a). Citation issued.
51. Plumbing installed; proper backflow devices
- comments: observed water leaking heavily from low temp dish machine and pooling onto floor. Instructed to repair machine and maintain in order to prevent possible pest harborage in dishwashing area.

- comments: observed previous core violations not corrected from report #2593659 on 4/30/24. They are as follows. . . 49)4-602. 13 observed shelving racks located in the walk-in cooling unit with excessive black and white moldlike buildup along with dried food debris. Instructed to clean and maintain or replace 49)4-601. 11(c)observed steam table splash guard in place on steam tables, beverage dispenser, and other equipment in front service in need of cleaning. Instructed to provide. 55)observed food debris on floors under heavy equipment in the front prep serving area, inside the walk-in cooler, behind stove equipment, and along the sidewalk in the freezer. Instructed to clean and maintain to prevent pest harborage. Instructed to correct. Priority foundation violation. #7-42-090. Citation issued.
