- comments: observed rear ice machine near meat department with a buildup of black and brown substance, dripping onto ice cubes stored below the ice bin. Instructed to clean, sanitize, and maintain. Priority foundation violation #7-38-005. Citation issued

- comments: noted wiping cloth on top of the cutting board in the meat department. Instructed to hold wiping cloths in proper sanitizing buckets with the proper sanitizing solution in between use

- comments: observed cutting boards in meat department with deep stains and cuts. Instructed to restore to a smooth and easy to clean surface by sanding and resealing or replace and maintain. .

55. Physical facilities installed, maintained & clean
- comments: noted holes in walls where baseboard tiles are broken and missing in the meat department, where a hand and 3 compartment sink is located. Also observed water damage to walls in the front office. Instructed to repair.
