16. Food-contact surfaces: cleaned & sanitized
- comments: observed low temperature dish machine at 0 ppm chlorine. Instructed manager to call machine in for service and to not use machine until it is repaired. Instructed manager must use 3 comp sink to temporarily wash, rinse, and sanitize all items. Item tagged until repaired. Priority violation 7-38-025.

22. Proper cold holding temperatures
- comments: observed tcs foods (rice) held at improper tempertatures between 50. 5 and 52. 4 degrees. Held inside 3 door prep cooler in basement prep area. Instructed manager that all tcs foods must be held at or below 41 degrees at all times. Manager voluntarily discarded all out of temperature tcs foods. Priority violation 7-38-005. Discarded 390lbs of rice with a value of $520.

33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed 3 door prep cooler in basement prep area to have an ambient air temperrature of 54 degrees. Found tcs foods (rice) held in cooler. Priority violation 7-38-005.

- comments: observed no thermometer present in glass door standing cooler in prep area. Provide a thermometer in all cooling units.

39. Contamination prevented during food preparation, storage & display
- comments: observed food storage bins stored on floor in walk in cooler in basement. All items must be off the floor at least 6 inches.

49. Non-food/food contact surfaces clean
- comments: cooler filter in basement prep area is dusty and full of debris. Maintain filter in good condition.

51. Plumbing installed; proper backflow devices
- comments: observed multiple leaks throughout the kitchen at time of inspection. Handsink in basement near prep area has a leak from below. 3 compartment sink in basement has leak from below. Sink next to dish machine hs leak from below. Observed leak from backflow prevention device attached to dish machine. Contact a plumber to investigate and repair all sources of leaks mentioned above.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed missing lids on trash receptacles in female bathroom in 3rd floor as well as employee bathroom in basement. Provide a tight fitting lid to receptables.

55. Physical facilities installed, maintained & clean
- comments: observed derbis and food residue build up between prep tables and prep coolers in basememnt prep area. Regularly clean and maintain area free from debris and food residue.

- comments: observed floor tiles throughout basement prep area to be cracked and missing. Repair and replace missing tiles, and maintain a cleanable surface.

- comments: observed debris and dust build up on ventilation hood filters above cooking area. Regularly clean and maintain area free of debris and dust.

- comments: observed certified food manager on site to not have allergen training. Complete allergen training and ensure all cfpms also have allergen training.
