- comments: 4-501. 11--observed a torn door gasket inside of the south kitchen 2 door stand-up cooler. Instructed to replace the torn door gasket and maintain.

- comments: 5-205. 15--observed low (hot) water pressure at handwashing sink next to the dishwashing 3-compartment sink;engineer is made aware. Instructed to repair plumbing issue and maintain.

- comments: 6-501. 114--instructed to detail clean and organize annex pre-k classrooms and main building therapeutic classrooms and remove unnecessary articles due to potential pest harborage and elevate all kept items off the floor at least 6' inches to facilitate cleaning and maintain.

Observe approximately 50 rodent(mice)droppings on the floor underneath the window wall heaters and in cluttered corners and in between storage cabinets inside the washroom of the eat-in-therapeutic classroom#302,#304 (main building),approximately 30 rodent (mice) droppings on the floor and shelfs inside the storage closet and on floors at corners of the teachers desk in adjacent office next to washroom inside the annex pre-k classroom#110 and approximately 15 rodent (mice)droppings on the floor behind the small blue students couch and on the window ledges in teachers connecting office between the annex pre-k classrooms#111,#107. Instructed to detail clean remove cluttered conditions and remove all rodent droppings from all affected areas. Additional pest control service is needed to eliminate the pest activity and maintain. Priority violation 7-42-090.
