- comments: observed approximately 60 small black live flies in the rear dry storage area, flying throughout in-use prep areas on top of surfaces of 3 compartment sink and employee toilet room. Instructed to contact pest control for service. Must eliminate fly activity. Priority foundation violation. #7-38-020 (a). Citation issued.

- comments: observed stacked soiled towels improperly air drying stored in toilet room. Instructed to remove to prevent pest harborage in said space.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed all plumbing fixtures in need of cleaning. Instructed to provide and maintain.
55. Physical facilities installed, maintained & clean
- comments: observed excessive clutter inside of toilet room. Instructed to remove all unnecessary items, organize and elevate at least 6 inches off floor. Noted floors in floors in need of cleaning in the dry storage room.
