38. Insects, rodents, & animals not present
- comments: rodent infestation observed on premises. Found 300 or more mouse droppings on the floor in the kitchen under the dishmachine, in corner area of kitchen behind rolling cart, behind the bar on the floor and bar cabinets/shelves, under the exposed handsink outside of kitchen, in basement areas on the floor in the small storage room, on the shelf in the storage room and on floors in corner areas of basement. Also observed droppings on the shelf above the stairs leading to the basement. Instructed to clean all affected areas and have pest control service establishment. Priority violation 7-38-020(a) citation issued for control of vermin and insects.

49. Non-food/food contact surfaces clean
- comments: must clean and maintain the following: exterior surfaces of all cooking equipment, bottom of fryers, wok station, prep tables, refrigeration units and freezers, storage containers, etc.

55. Physical facilities installed, maintained & clean
- comments: instructed to clean and maintain the following: walls around and behind cooking equipment, floors under around and behind all equipment in kitchen, storage areas of basement and walk-in cooler/freezer. Also instructed to clean the ceiling vents in kitchen to remove dust observed, clean interiors of light shields to remove dust and other matter inside of shields.

- comments: observed a floor drain in basement with no cover; instructed to provide and maintain.

56. Adequate ventilation & lighting; designated areas used
- comments: instructesd to clean the vents in the hooded system above the cooking equipment and maintain.
