2. City of chicago food service sanitation certificate
- comments: observed no certified manager from 12:35pm to 1:55pm while tcs foods such as chicken were prepared. At 1:55pm certified manager peblo velazquez arrived on site. Instructed to maintain employee holding city of chicago foodservice manager certificate on site at all times. Priority foundation violation 7-38-012. Citation issued.

43. In-use utensils: properly stored
- comments: observed ice scoop laying on top of ice in rear area. Instructed manager to store ice scoop handle up above ice to prevent contamination.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered receptacles inside of womens toilet rooms. Instructed to provide.

55. Physical facilities installed, maintained & clean
- comments: observed debris on floor in dry storage area. Instructed manager to clean and maintain.

58. Allergen training as required
- comments: observed food service sanitation manager without food allergen training certificate. Instructed to meet requirement.
