- comments: no valid city of chicago sanitation certificates on the premises. Must provide a valid city of chicago sanitation certificate, and such a person be on-site at all times when foods are prepared/served. Priority foundation violation 7-38-012 citation issued.
38. Insects, rodents, & animals not present
- comments: no pest control log book on site or required documentation on site. Must provide. Priority foundation violation.
49. Non-food/food contact surfaces clean
- comments: shelving under the front prep hot holding unit noted with excessive food debris. Must clean and maintain. ----must not use tin foil as liner for shelving at the basement prep. ----raw wooden shelving under the basement microwave oven must be painted and sealed.

- comments: missing light shield end caps throughout the basement. Must replace/provide.
