Food Safety Report: Chipotle.

Took out chicken bowl, rice, salsa, black beans ,cheese, sour cream on Sat, 12/19/15. Completely sick by 12:30pm on Monday, 12/21. Sweating profusely, faint, lightheadedness, nausea, stomach cramps. Had to leave work. Home next day, sicker than I've ever been in my life! | Symptoms: Diarrhea, Nausea, Fever
