- comments: observed no person-in-charge present with city of chicago certified manager certificate.

- comments: observed no certified food manager on site or valid documentation while (tcs foods) (chicken, burgers, pizza etc. )are being prepared/served to the customers. Instructed person in charge, a certified food manager with city of chicago foodservice sanitation certificate must be present while preparing handling and or serving (tcs foods). Priority foundation violation 7-38-012, citation issued.

- comments: observed no hand drying provisions at the front and rear hand washing sinks. Instructed to provide and mantain. Priority foundation violation 7-38-030(c) citation issued

- comments: observed cooked rice at 106. 4f sitting on a shelf. Instructed to keep tcf foods for hot holding at 135f or above and maintain. Priority violation 7-38-005 manager voluntarily discarded 4lbs of rice valued $20. 00. Citation issued.

- comments: observed approximately 50 mice droppings under dry storage racks and inbetween clutter articles stored in the rear. Instructed to clean and maintain. Suggested to contact pest control company to treat for rodents. Priority foundation violation 7-38-020(a) citation issued

- comments: observed shelves in the walk in cooler rusted. Instructed to clean or replace and maintain.
