- comments: observed no hot water at hand washing sinks in girls and boys washroom near kitchen (73. 2f-70. 9f). Instructed manager to provide hot water at all hand washing sinks for proper hand washing. Priority 7-38-030(c) citation issued

- comments: observed 1/4" gap at bottom of left front entrance door. Instructed manager to seal all outer openings on door to prevent pest entry.

- comments: observed cup used as dispensing utensil for ice. Instructed manager to provide proper ice dispensing utensil with handle stored above ice to prevent contamination.

- comments: observed milk crates used as shelving inside walk-in freezer. Instructed manager to provide adequate shelving for storage.

- comments: observed openings around pipe fittings along lower walls in kitchen near hand washing sink, damaged and missing wall tiles and baseboards along walls in kitchen, peeling paint along walls in rear storage area near walk-in freezer and dining area. Instructed manager to repair/replace and maintain.

- comments: previous core violation not corrected from report #2603769 on 10-1-24:38-observed the rear exit door in the room with the walk-in freezer area not rodent proofed at the bottom. Instructed to rodent proof said door and maintain. Priority foundation 7-42-090 citation issued
