- comments: observed no city of chicago certified food service manager on site wile tcs foods are being handled (reheating chicekn wings, chicken taquito, pizza etc). Instructed above to operate with a city of chicago certified food service management at all times food is handled. Priority foundation violation 7-38-12. Citation issued.

- comments: observed no written employee health policy on premises at this time of inspection. Management instructed to provide a signage employee health policy for each employee. Priority foundation violation 7-38-010.

- comments: observed several items stored on floor inside walk-in cooler and rear food storage area. Must elevate items of the floor and maintain at all times.
55. Physical facilities installed, maintained & clean
- comments: observed dirt and food debris on floor around utility mop sink located in rear food storage area. Must clean and maintain at all times. Observed heavy accumulation of dust build up on ceiling ventilation vent inside the unisex employee washroom. Must clean and maintain at all times.
