16. Food-contact surfaces: cleaned & sanitized
- comments: obsevred internal panel of ice mchine with pink slime mold-like build up dripping on to ice used for human consumption. Instructed to clean equipment and maintain clean food contact surfaces. Priority foundation violation. #7-38-005. Citation issued.

38. Insects, rodents, & animals not present
- comments: observed approximatley 50 small rodent droppings on floors behind ice machine, along radiator vents. Behind all heavy equipment in prep under 3 compartment sink basins, inside walk in coolers used fro dry storage and under sheling units in dry storage near back exit door, on floor under gate connected to dining room area and inside rolling cart steam table used for serving residents. Instructed to contact pest control for additional service, remove all droppings and sanitize all said areas. Must eliminate pest activity. Priority foundation violation. #7-38-020 (a). Citation issued.

44. Utensils, equipment & linens: properly stored, dried, & handled
- comments: instructed to invert plates stored on steam table rolling cart in order to prevent posible contamination.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed all plumbing fixtures in employee toilet room in need of cleaning. Instructed to clean and maintain.

55. Physical facilities installed, maintained & clean
- comments: observed food debris behind all heavy equipment wher bait boxes are stored, noted pooling water under prep table where coffee machines is stored. Instructed to remove and clean floors.
