38. Insects, rodents, & animals not present
- comments: 6-501. 111 observed evidence of pest activity on site. Observed about 10 rodent droppings located inside an unused walk-in cooler (left side). Observed about 10 rodent droppings located between the wall and the oven on the food prep area cooking line. Instructed to eliminate live rodent activity, clean and sanitize affected areas, and recommended contacting pest control for a service. Priority foundation 7-38-020(a).

49. Non-food/food contact surfaces clean
- comments: 4-601. 11(c) observed lower shelves with dirt debris under the hot holding unit. Instructed to clean.

51. Plumbing installed; proper backflow devices
- comments: 5-205. 15 observed one of two hand washing sinks not turning on in the basement girl's toilet room. Instructed to repair.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: 5-501. 17 observed no covered receptacles inside of the employee toilet rooms and in the basement and 3rd floor girls toilet rooms. Instructed to provide.

55. Physical facilities installed, maintained & clean
- comments: 6-201. 11 observed a missing floor tile on the floor located by the 3-compartment sink. Observed the walk-in cooler door rotting at the bottom with excessive rust. Instructed to repair and make said surfaces smooth and easily cleanable.

- comments: 6-501. 114 observed oven racks being stored directly on the floor by the food prep/cooking line. Instructed to elevate everything at least 6 inches.

- comments: 6-202. 11 observed a broken light shield located by the south windows above a prep table. Instructed to replace broken light shield.
