- comments: 2-101. 11 observed no city of chicago certified food manager on site during time of inspection while preparing and serving tacos, chicken, pizza puffs, sliced tomato, and polish. Instructed to provide at least one with a valid original certificate. Priority foundation violation. #7-38-012. Citation issued.

- comments: 4-602. 13 observed rolling carts, shelf containing slushy machines, cooling units, and frying equipment with dried food debris on interior and exterior surfaces. Instructed to clean and maintain.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: 5-501. 17 instructed to provide a garbage can with a lid inside the toilet room.

55. Physical facilities installed, maintained & clean
- comments: 6-501. 114 observed trash bags, boxes, and grease-filled containers stored in rear storage area where mop sink is located. Instructed to remove unnecessary items and maintain the premises.
