2. City of chicago food service sanitation certificate
- comments: no city of chicago certified food manager on site during the inspection while the temperature control for safety foods (chicken, beef and fish) has been prepared and served. Instructed a city of chicago certified food manager must be on site at all times foods are being prepared and served. Priority foundation violation 7-38-012. Citation issued

5. Procedures for responding to vomiting and diarrheal events
- comments: observed no written procedures or supplies for responding to vomiting or diarrheal events. Supplies must include adequate sanitizer that eliminates norovirus. Instructed facility to provide clean up policy and supplies to properly handle vomit and diarrheal events on site. Priority foundation violation #7-38-005. Citation issued.

16. Food-contact surfaces: cleaned & sanitized
- comments: observed low temperature warewashing machine (in use) located bar area not properly sanitiatzing. The chlorine sanitizing concentration was observed at 0ppm at time of inspection. Instructed not to use until equipment is properly chlorine sanitizing at a minimum of 50 ppm. Tagged machine. Priority violation #7-38-025. Citation issued.

22. Proper cold holding temperatures
- comments: observed improper temperatures of the following items in cold holding unit, in rear prep: 5# chicken at 58. 9f, 1# tilapia at 60. 2f, 2# salmon at 61. 2f and 4# of potatoes at 62. 4f. Must have cold holding foods at 41. 0f or below. Manager immediately and voluntarily disposed of 12lbs of products worth $250. 00 through denaturing process. Priority violation #7-38-005. Citation issued.

33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed temperature of 2-door reach in prep cooler to be improper at 61f in rear prep. Cooler must maintain a proper cold holding temperature of 41f or below at all times. Reach in cooler tagged and held for inspection by the chicago department of public health and may not be used until uniut is re-inspected by cdph and maintaining a proper cold holding temperature for 41f or below. Priority violation #7-38-005. Citation issued.

38. Insects, rodents, & animals not present
- comments: observed pest activity with approx. 15 small flies noted during inspection. Small fly activity was at bar and rear prep, and dishwashing area. Instructed to consult pest control as additional services are needed to minimize or eliminate activity prior to re-inspection. Priority foundation violation #7-38-020(a). Citation issued.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed cutting boards in the rear prep area with open seams and dark stains. Instructed to refinish or replace damaged or stained cutting boards and maintain at all times.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed facility without a chemical test kit for sanitizer or for ware washing area and low temp dishwashers, informed person in charge a test kit that accurately measures the concentration of sanitizing solution must be provided. Priority foundation violation #7-38-005. Citation issued.

51. Plumbing installed; proper backflow devices
- comments: observed drainpipe leaking under 3 compartment sink from middle sink in dishroom. Instructed to repair leaking drainpipe.

55. Physical facilities installed, maintained & clean
- comments: observed missing ceilings tile panel at the utility sink. Instructed to repair, replace missing panels and/or replace the stained ceiling tiles.

58. Allergen training as required
- comments: observed no city of chicago food certified manager with no proof of allergen training certificate. Instructed all city of chicago certified food managers obtain this certificate.
