- comments: noted torn rubber gasket inside the prep service cooler at the 1st floor prep area and lid of chest freezer at the basement food/dry storage in poor repairs (ripped off). Instructed to repair and maintain and/or replace.

- comments: noted the dipper well sink in poor repairs with water leaking from the base of the sink. Instructed to repair and maintain.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: noted washrooms on-premise with garbage receptacles and no lids. Instructed to provide trash cans with lids at all women washrooms and unisex washrooms.
55. Physical facilities installed, maintained & clean
- comments: noted the following:- --cracks/peeling paint on walls leading to the basement food/dry storage area --cracked tiles on wall base leaking th the basement food/dry storage area. --floor along wall base especially at the basement food/dry storage area. Instructed to repair cracked tiled, clean and repaint peeling paint on the walls and clean and maintain floors.

- comments: noted clutter on shelves, floors unused equipment and in storage areas at the basement food/dry storage. Instructed to detail clean, organize and remove all unnecessary or unused articles and equipment to prevent hiding places for pest.
