- comments: observed pest activity in facility. Observed approximately 30 mouse droppings in facility. Observed mouse droppings in small storage room next to three compartent sink and in teacher's lounge. Instructed to eliminate rodent activity, remove mouse droppings and clean affected areas. Observed 10 ants in window and counter top of cabinet next to window in room 128. Instructed to eliminate ant activity and clean affected areas. Recommended licensed pest control company service facility on or after 4/13/2026. Priority foundation violation 7-38-020(a) no citation issued

- comments: observed opening at base of rear exit door of prep area with utility sink. Instructed to seal all outer openings to prevent pest entrance.

- comments: observed damaged gasket on left door of two door refrigerator across from serving line. Instructed to repair or replace damaged gasket on left door of two door refrigerator across from serving line and maintain.

- comments: observed ice buildup on rear upper interior in middle above top shelf in two door freezer across from serving line. Instructed to remove ice buildup from rear upper interior in middle above top shelf in two door freezer across from serving line and maintain.

- comments: observed leaking from drainpipe of sanitizer compartment of three compartment sink. Instructed to repair leaking drainpipe of sanitizer compartment of three compartment sink and maintain.

54. Garbage & refuse properly disposed; facilities maintained
- comments: observed recycling in dumpster overflowing and preventing lid from closing which created a large opening in recycling dumpster. Instructed to keep trash and recyclables covered and maintain tight fitting lids on dumpster. Priority foundation violation 7-38-020(b) no citation issued

- comments: observed non working lights in both walk in freezers. Instructed to replace or repair non working lights in both walk in freezers to provide light in both walk in freezers and maintain.

- comments: at time of inspection facility provided expired food handler certificate for food handler. Instructed to provide valid food handler certificates for all food handlers.
