2. City of chicago food service sanitation certificate
- comments: observed facility food handlers handling�time and temperature control for safety(tcs) foods without original city of chicago food service manager and certificate on site. Informed person in charge of city of chicago original food service certificate must remain posted on site and certified manager on site during serving and operating hours. Priority foundation violation #7-38-012. Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no paper towel at the hand-sink in front of the prep area. Informed person in charge that paper towels must be available at hand sinks at all times. Food handler obtained paper towel for hand sink during time of inspection priority foundation violation #7-38-030(c). Citation combined with the above violation

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed handsink in front prep area not stocked with hand soap. Informed person in charge that hand soap must be available at handsinks at all times. Food handler obtained hand soap for hand sink during time of inspection. Priority foundation violation #7-38-030(c). Citation issued.

16. Food-contact surfaces: cleaned & sanitized
- comments: observed ice machine located in rear prep/ dish with a build-up of black and pink slime substance, dripping onto ice cubes stored below the ice bin. Ice is used for human consumption. Also, black slime on the exterior of the unit on the ice bin. Interior plate of ice bin with excess white and pink slime at ice dispenser nozzle of same unit. Instructed to clean, sanitize, and maintain. Priority foundation violation #7-38-005. Citation issued

39. Contamination prevented during food preparation, storage & display
- comments: observed food preparation area (front counter) without proper protection from contamination. Must install a barrier to separate customer area from prep area. Must provide barrier along counter (sneeze guard) to protect equipment from customer contamination.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed milk crates being used as shelving throughout the prep and storage areas. Must remove and provide shelving that is 6" off the floor and provides floor accessibility to ensure adequate facility cleaning.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed facility without quat chemical test kit for sanitizer or for ware washing area. I informed the person in charge that a test kit that accurately measures the concentration in mg/l of sanitizing solution must be provided. Priority foundation violation #7-38-005. Citation issued.

51. Plumbing installed; proper backflow devices
- comments: observed inadequate disposal of clean water; pipe in dish area dripping clean water into 1st sink basin at 3 compartment sink. Instructed to redirect the disposal of clean water to the floor drain according to law. Priority violation #7-38-030(c). Citation issued.

52. Sewage & waste water properly disposed
- comments: observed two 3-compartment warewashing sink without approved grease trap located in rear dish/prep area. All sinks in which food, utensils, and equipment are washed shall be connected to also have a catch-basin or be equipped with an approved grease interceptor. Instructed person in charge to obtain, install and maintain at all times.

- comments: observed a/c unit condenser covered in plastic that is hanging down above 3 compartment sink, instructed to repair and remove plastic that is hanging and maintain.

- comments: observed no proof of training for all food handlers. Instructed to provide and maintain records at all times.

- comments: observed most recent inspection summary report not posted from inspection #2463913 dated 12/17/2020. Instructed must post the most recent inspection summary report in plain view at all times. Operator provided with today's summary during inspection. Priority foundation violation 7-42-010(b).
