- comments: observed no employee health policy on site. Instructed to provide an employee health policy that includes a signed acknowledgement from each employee. Priority foundation 7-38-010. Citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hand washing sink conveniently located near the 1-compartment sink/coffee prep area. Instructed management that a handwashing sink must be located to allow convenient use by employees in all food prep areas. Must install a handwashing sink in said area. Priority foundation 7-38-030(c). Citation issued.

- comments: observed no handwashing signage at the bar or basement handwashing sinks. Must provide.

- comments: observed an incomplete consumer advisory for raw/undercooked foods - the advisory is missing a disclosure statement. Instructed management to add a disclosure statement to the current advisory that says 'items can be served raw or undercooked or contain undercooked ingredients' and remove the asterisks from fully cooked menu items such as chicken and soups. Priority foundation 7-38-005.

- comments: observed no label on the bulk food storage containers in the basement dry storage area. Must label and maintain.

- comments: observed heavy ice build up inside the 1-door true freezer located on the 1st floor prep area. Must remove ice and maintain.

- comments: observed no covered waste receptacles inside the 1st floor unisex washrooms. Must provide.

- comments: observed heavy dust and dirt build up on the ventilation hood located over cooking equipment. Instructed to clean and maintain.
57. All food employees have food handler training
- comments: unable to locate food handler certificates. Instructed management that all food employees must complete food handler training and keep certificates on site.
