- comments: observed wet wiping cloths on several prep area surfaces. Instructed to properly store wiping cloths in a sanitizing solution between uses to prevent contamination of equipment and food.

- comments: observed rusty surfaces on bottom of prep tables in rear prep area. Must repaint or replace.

- comments: observed damaged door gaskets at the 2-door tall refrigerator in rear prep area. Must repair and maintain at all time.

- comments: observed leaking faucet base at the 3-compartment sinks in front and rear dish washing area. Must repair and maintain

- comments: outside garbage area not maintained. Observed outside garbage dumpsters overflowing preventing lid from closing properly also observed garbage and trash on ground surrounding dumpsters. This is a food source for rodents and other pest. Must maintain garabe area adeqaute maintain at all times. Priority foundation violation 7-38-020(b) citation issued.

- comments: observed dirt and food debris on floor underneath food storage shelving in rear food prep area. Must clean and maintain. Observed dust and dirt build up on ceiling ventilation vent above 2-door tall refrigerators in rear prep area. Must clean and maintain.

- comments: observed no proof of allergen training for the certified food manager. Instructed any persons with the city of chicago food sanitation certificate to comply with completion of training. Must provide documentation.
