- comments: observed no employee health policy on site. Must provide written employee health policy for all employees. Priority foundation violation. 7-38-010.
5. Procedures for responding to vomiting and diarrheal events
- comments: observed no vomit and diarrhea clean up kit/procedure on site. Must provide written procedure for the clean up of vomit and diarrheal events, and corresponding equipment outline in procedure. Priority foundation violation 7-38-005.
6. Proper eating, tasting, drinking, or tobacco use
- comments: observed employee drinks on prep table and stored inside prep cooler. Must keep all employee beverages in a designated area.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed prep table and dough rolling machine in rear storage room without required hand washing sink. Must install a dedicated, exposed hand washing sink with hold/cold plumbed water; soap/paper towels, and a hand washing sign. Priority foundation violation 7-38-030(c).

- comments: observed heavily soiled large mixer in prep area. Must detail clean and sanitize equipment. Must maintain same.

- comments: observed large mixer in prep area. Must provide a written, approved method (and demonstrate) to properly wash, rinse and sanitize all clean in place equipment.

- comments: observed excessive accumulation of grease on donut fryer. Must detail clean and sanitize. Must maintain same.

- comments: observed slow draining 3 compartment and hand sink in rear of prep area. Must repair and maintain same.

- comments: observed waste water leaking from pipe under 3 compartment sink on to floor. Instructed must convey all waste water properly. Must repair. Priority violation 7-38-030(c). Citation issued.

- comments: observed cardboard used as a floor covering in front fryer area. Must remove. All surfaces must be non-porous, smooth and easily cleanable.
