Report date: Oct 26, 20215. Procedures for responding to vomiting and diarrheal events
- comments: found missing items for vomit spill kit. Sanitizer rated for norovirus, disposable aprons, gloves, etc. Priority foundation. No citation was issued. 7-38-005 must provide.
38. Insects, rodents, & animals not present
- comments: observed small fly activity in basement (approximately 20 small flies) in all areas containing standing water, including dry storage area and prep area. Repair all leaking conditions. Remove all standing water. Clean all affected areas. Dry thoroughly and contact pest control. Priority foundation violation 7-38-020(a)
39. Contamination prevented during food preparation, storage & display
- comments: observed dry storage and food-related items stored under sewer lines in the basement. Relocate all dry storage, chest freezer, and any food-related items from under sewer lines or install a smooth, easily cleanable ceiling to protect all food and food-related items or encase all pipes for protection.
51. Plumbing installed; proper backflow devices
- comments: observed 3 compartment sink in basement leaking under far-right compartment. Repair and maintain. Observed drain for 3 compartment overflowing onto floor when sink is filled, and compartment is drained. Clean the drain so that water drains properly and is maintained.

- comments: observed standing water in the following areas of the basement. In east corner of dry storage room and leaking into non working walk-in cooler used for dry storage. Observed standing water next to chest freezer and around under behind ice machine and 3 compartment sink in basement. Clean up all standing water and maintain floors in a clean, dry condition.

- comments: found excessive clutter in basement storage areas. (old equipment, etc. )must be clean and organized.

- comments: observed standing water in basement on floor and dripping from ceiling into bucket. Observed wall and ceiling exposed. The leaking condition must be repaired. The area needs to be properly dried, and the walls and ceiling need to be restored to a smooth, easily cleanable condition.
