- comments: observed facility not meeting chemical test kit concentration level requirements in setup 3-compartment sink for compartment with quats sanitizer. Noted concentration level of compartment with quats sanitizing solution measured at 0ppm. The person in charge was informed of the proper sanitizing concentration levels. The sink was emptied. Priority violation #7-38-025. Citation issued
16. Food-contact surfaces: cleaned & sanitized
- comments: observed a white panel inside the ice machine that was filled with ice and was covered with a black mold-like substance. Instructed manager to wash, clean, sanitize, and maintain. Ice is used for human consumption. Priority foundation violation. 7-38-005. Citation issued.
28. Toxic substances properly identified, stored, & used
- comments: observed raid roach killer spray, located under 3 compact sinks at bar. Informed manager that containers previously used to store poisonous or toxic chemicals/materials may not be used to store, manager corrected violation during inspection. Manager voluntarily discarded priority violation # 7-38-005 citation issued.
38. Insects, rodents, & animals not present
- comments: observed a prohibited animal (dog) on site in the facility. The person in charge was made aware that such animals are prohibited unless they are service animals. Instructed to move to outdoor patio area and remove animal from the facility and maintain at all times. Dog was removed during inspection. Priority foundation #7-40-060. Citation issued.
48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed no (qac) chemical test kit available during inspection. Instructed manager to provide test kit for proper concentration in sanitizing solutions. Priority foundation 7-38-005 citation issued.
55. Physical facilities installed, maintained & clean
- comments: observed missing and damaged floor at bar also noted toilet room has 2 inch hole on floor, instructed to repair to create a smooth and easily cleanable surface. Must maintain at all times.
