- comments: found no procedure/plan and kit for responding to vomiting and diarrheal events. Instructed facility to develop and maintain a procedure/plan and to maintain any appropriate supplies on site. Priority foundation violation 7-38-005 no citation issued.

38. Insects, rodents, & animals not present
- comments: observed evidence of mice droppings in different location of kitchen: 6 droppings on shelf under prep table,8 droppings between food cans under the shelf by the cooking equipment, 10 or more on shelves at north site of the cooking equipment, 7 droppings on shelf at south end of cooking equipment. Mice droppings(over 50 droppings)were absorbed inside storage area in dining area. Instructed to clean and sanitize mentioned area priority foundation violation:7-38-020(a),citation issued.

- comments: must store wiping cloths in prep areas in sanitizer bucket between uses to prevent cross contamination. Instructed to maintain.

- comments: must not use plastic grocery bags as food containers inside the box freezers and reach-in coolers. Must not use cardboard boxes as food containers. Must use washable containers or food grade disposable plastic bags.
