- comments: noted bulk dry seasonings and grains in unlabled containers. Instructed to provide labels of common names for all ingredients removed from its original container.
38. Insects, rodents, & animals not present
- comments: observed pest control logbook not maintained with reports per service agreement. Person in charge says pest control comes every six months. Reports in log are dated 10/3/23 and 6/25/24; instructed to provide proof of service. All retail food establishments must maintain a logbook that shall be kept for a period of twelve (12) months and include a copy of the service tickets and the pest control company’s illinois department of public (idph) health license number. Priority foundation violation #7-38-020(c). "

- comments: observed evidence of pest activity. Noted 4 live roaches in pest control logbook 1 live roach on wall in lobby and 2 live roaches on the floor under service table. Instructed to consult pest control as additional services are needed to eliminate activity prior to re-inspection. Priority foundation violation #7-38-020(a). Citation issued.

- comments: noted slow drain at utility sink. Instructed to address clog, repair and maintain.

- comments: noted clutter throughout kitchen and prep area. Instructed to organize, remove any unnecessary items and elevate all kept items a minimum of 6 inches from the ground on proper shelving that is smooth and easy to clean
58. Allergen training as required
- comments: observed no proof of allergen training for all certified food managers. Instructed any persons with the city of chicago food sanitation certificate must comply with completion of training and provide and maintain documentation on site at all times.
