1. Person in charge present, demonstrates knowledge, and performs duties
- comments: observed no one on site demonstrating food safety knowledge 2-102. 14

2. City of chicago food service sanitation certificate
- comments: obsreved no city of chicago certifed food manager on site while preparring cooking and holding of food including pork and beef. Observed no original certificate posted. Priority foundation violation 7-38-012

32. Variance obtained for specialized processing methods
- comments: observed no haccp plan and variance for smoking meat and vacuum packaging machine on premises. ---alo observed assorted food items inside coolers vacuum packaged with no proof of haccp or variance per health code requirement. Instructed management discontinue the vacuum packaging process until a variance and haccp are approved by the health department. Priority foundation violation #7-38-005. Citation issued.

55. Physical facilities installed, maintained & clean
- comments: observed 1st floor cooking area with door on far end leading to large storage rooms. Rooms were packed full of old equipment. All clutter must be removed from the storage rooms and all remaining items must be organized in a way that is 6 inches off the floor and away from the walls for cleaning and pest control access. Maintian storage areas.

57. All food employees have food handler training
- comments: observed no certificates for food handlers. All food handlers must show proof of food handler certificate.

58. Allergen training as required
- comments: observed no proof of allergen training for certified food manager. Must provide and maintain.
