- comments: observed no handwashing signage at the handsink. Instructed person in charge to provide handwashing signage at all handsinks.

- comments: observed excessive build up of pink and black mold-like substance on interior of ice machine dripping onto ice. (ice used for human consumption, bar drinks). Instructed person in charge to discard ice, detail clean machine, and remove all mold like substance and maintain. Prioirty foundation #7-38-005. Citation issued.

- comments: observed approximately 100 rat droppings in the following areas; under basement stairs, under shelving in basemen across from stairs, in all corners of bar. Instructed person in charge to remove all rat droppings and sanitize all areas. Priority foundation #7-38-020(a). Citation issued.
