2. City of chicago food service sanitation certificate
- comments: found no original city of chicago sanitation certificate on premises while handling tcs foods. (handles sausage, eggs. ) manager provided scanned copy of certificate. Priority foundation violation, 7-38-012. Citation issued. Must provide original certificate for certified manager on site during hours of operation.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed handwashing sink at front prep area by donut display area to be not accessible for use as it was blocked by metal trays stacked on top of the splash guards of the hand sink. Instructed manager that all hand sinks must be maintained accessible for use by employees at all times. Priority foundation violation, 7-38-030(c). Citation issued. Manager removed all trays and made sink accessible.

39. Contamination prevented during food preparation, storage & display
- comments: observed food (milk crates and boxes of creamers) stored on the floor in the walk in cooler. Instructed manager to provide additional shelving or elevate all food items at least 6 inches off the floor in walk in cooler.

51. Plumbing installed; proper backflow devices
- comments: observed leak at base of faucet of the 3 compartment sink located in the ice cream prep area at front. Observed hand sink at rear prep area of facility (next to walk in cooler) to have a loose cold water/right ride handle, causing the water to not be able to be turned off fully. Hot and cold water is still available. Instructed manager to service base of faucet and handle of sink and maintain in good repair.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered trash can in the employee bathroom located on the right side. Instructed manager that all unisex bathrooms must have a covered trash receptacle available.

55. Physical facilities installed, maintained & clean
- comments: observed significant ice build up on the floor of the walk in freezer in the back corner. Instructed manager to remove and maintain area free of excessive ice build up.

56. Adequate ventilation & lighting; designated areas used
- comments: observed employee personal items such as jackets stored on storage shelves used for single service items in the rear dry storage area. Instructed manager to designate area for employee personal items and for employees to use designated area away from single service and food items.
