- comments: instructed to provide handwashing signage at all handsinks (washrooms).

- comments: observed side entrance door(addison side) with opening approximately 1/4' inch at bottom of door. Instructed to make door tight fitting all across the bottom to prevent pest entry.

- comments: observed approximately 20 live small flies on the walls at the front food prep area (baskin robbins side) and walls and ceiling leading into the washrooms. Instructed to detail clean and sanitize affected areas and pest control service is now reccomended. Priority foundation violation 7-38-020(a),citation issued.

- comments: observed stained cutting board on top of prep cooler at front prep area. Instructed to resurface or replace the cutting boards.

- comments: observed dim lighting inside the walk-in-cooler. Instructed to provide brighter bulb and maintain adequate lighting at all times.
