- comments: observed evidence of 10 -rat droppings on floor next to tool table & behind hot water tank in tool room,appx 50 or more on floor under electrical box,on shelving units & electrical outlets in dry food storage area,on ledge and on floor in between massive clutter,under tables in additional side storage area possible rodent harborage area. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 11/2/20. (priority foundation 7-38-020(a)(citation issued)

- comments: observed a plastic bag with wet chemical test strips above 3- compartment sink and pic showed me a bag of discolored strips. Instructed to provide appropriate test strips with color chart to test solution ppm's. Must provide and maintain. (priority foundation 7-38-005)(citation issued)

- comments: must clean grease and food dberis build up from fryer cabinets and wheel bases, excessive grease on shelving units in front prep area,prep tables,hot holding units,reach in coolers,dry food storage shelves,encrusted exterior of large dry food storage containers,service counter shelving units.

Instructed to provide receptacle with covers to unisex and women's toilet room. Maintain same. #55-the entire west side of the facility (the dining area on back) with massive amount of clutter. Instructed to eliminate all unnecessary items and organize the area. Maintain same. Must provide and maintain. (priority foundation 7-42-090)(citation issued)
