10. Adequate handwashing sinks properly supplied and accessible
- comments: must repair or replace observed low hot & cold water pressure at hand sinks in men's rear toilet rooms,water doesn't turn off at hand sinks in women's rear toilet room

36. Thermometers provided & accurate
- comments: must provide thermometers for all refrigerators.

37. Food properly labeled; original container
- comments: must label food storage containers (squeeze bottles/bulk containers) when food is not in original package.

38. Insects, rodents, & animals not present
- comments: observed evidence of appx. 100 or more mice & rat droppings under rear hand sink,dry food storage area on wood ledge & shelving units,on floor in between massive clutter by furnance,under lockers,on seating booth and on floor underneath,in walk in cooler shelving units and floor,on service counter shelving units. 4-6 live large flies flying around and appx. 25 or more live ants crawling on floor & on under desk area,on walls at hand sink. Must remove droppings clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 9/27/24. (priority foundation 7-38-020(a)(citation issued)

- comments: must eliminate crates used for storage near front service counter, dry food storage area and where needed.

- comments: must clean excessive grease and food debris build up interior & exterior of fryers,pipes of cooking equipment,prep tables,rolling carts, food storage containers,walk in cooler shelving units,all hand sinks ,1 & 3 comp sink basins and faucet handles,service counter shelving units and storage areas,wheel bases and legs of equipment,used and unused equipment.

- comments: must repair or replace leaky faucets at 1- comp & 3- comp sinks.

- comments: must remove,elevate and organize unnecessary articles from front service counter shelving units,rear hallway near rear door and where needed.

- comments: must repair or replace broken and uneven not smooth concrete flooring ,missing and damaged floor tiles throughout including walk in cooler,stained & missing ceiling tiles including toilet room,damaged ceiling in rear hallway,holes in walls and door of toilet room in front prep area,damaged wall bases. Must remove excessive caulking from walls where needed.

- comments: observed no food handlers training for employees must provide and maintain.

60. Previous core violation corrected
- comments: previous core violations from report #2579847 8/9/23 not corrected #49- 4-602. 13 : must clean the interior of the coolers and freezer to remove spills and/or debris. Also defrost and clean the small deep freezers in the prep area. #55-6-501. 114 : observed excessive clutter thru out the entire rear of the facility; buckets being re-used, construction materials, and old equipment. Instructed to remove all unnecessary clutter and elevate everything at least 6 inches from the ground to prevent harborage. #55-6-501. 12 : floors through out need detail cleaning to remove spills, debris and grease. Also clean the walls, ceiling and counter tops. #56-. 6-501. 14 : observed the ventilation system above the cooking equipment dirty. Must clean and maintain.
