- comments: observed no manager with a valid/physical city of chicago food certificate on site at time of inspection. Manager provided image of certificate, but no physical city of chicgao certificate present at facility. Instructed current person in charge that a manager with a valid city of chicago food certificate must be present during all hours of operation. Priority foundation violation, #7-38-012. Citation issued.

3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: found no employee health policy/training on site. Instructed to provide a signed employee health policy for each employee. Priority foundation violation, 7-38-010. Citation issued.

- comments: observed single service items, containers, and boxes stored on the floor in the elevated/over the walk in cooler storage area accessed by ladder. Instructed to elevate all items at least 6 inches off the floor.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed no sanitizing test strips (chlorine) available for three compartment sink for any chemical sanitizers used on site. Instructed to provide. Priority foundation violation, 7-38-005. Citation issued.

56. Adequate ventilation & lighting; designated areas used
- comments: observed employee personal items and jackets stored hanging on storage racks in rear preparation and storage area. Instructed manager to designate an area for employees to store all personal items away from food and single service items and prep tables.

60. Previous core violation corrected
- comments: previous core violations not corrected from 01/26/2024, report #2588695: #39) observed ready made boxes stored on floor in front display area. Instructed to properly store 6" off the floor to prevent contamination and for easy access on cleaning. #51) backflow preventer device not located behind the ice machine infront prep area. Instructed to install so that it can be located to be serviced and maintained. #52) instructed to install grease box connected underneath the 3 compartment sink (right side). #57) food handlers with no proof of food handler training certificate. Instructed to provide certificate for each employee. Priority foundation violation: 7-42-090, citation issued.
