- comments: observed exposed handwashing sink in rear dish area made inaccessible by tea pot being stored in handwashing sink. Instructed to maintain all handwashing sinks in an open and accessible manor. Priority foundation violation 7-38-030(c). Citation issued.

- comments: observed the following repeat priority foundation violation not corrected from report #2581849 dated 9/18/2023: 'observed 9 live roaches in corner of rear prep area near stairs and where loaves of bread area stored. Instructed to remove pests, receive additional pest control service, and maintain facility free of pest activity. Priority foundation violation 7-38-020(a). Citation issued. ' observed 10 live roaches throughout bar, prep area, and dish area. Instructed to correct and maintain facility in good condition. Priority violation 7-42-090. Citation issued.
