- comments: observed no employee health policy on premises at this time of inspection. Instructed management to have a signage employee health policy for each employee. Priority foundation violation 7-38-010 citation issued.
25. Consumer advisory provided for raw/undercooked food
- comments: menu does not disclose and inform consumers the specific menu items that are raw or under cooked and a potential hazard of consuming such food. Foods such as raw sushi. Must provide a consumer advisory on all menus and disclose and remind of all such foods. Priority foundation violation no citation issued
37. Food properly labeled; original container
- comments: must label food storage containers when food is not in the original container.
40. Personal cleanliness
- comments: observed employees not wearing hair restraints while food is being prepared and served. Must wear hair restraints at all times food is being prepared and served.
41. Wiping cloths: properly used & stored
- comments: observed wiping cloths stored on top of prep tables and hand washing sink in rear kitchen area. Must properly store wiping cloths in a sanitizing solution when not in use.
45. Single-use/single-service articles: properly stored & used
- comments: observed items stored on floor throughout rear kitchen food preparation area. Must elevate items 6 inches of the floor and maintain.
51. Plumbing installed; proper backflow devices
- comments: observed no backflow preventer device at the utility sink located inside the unisex employee washroom. Must nstall a back flow preventer device at the utility sink and maintain.
55. Physical facilities installed, maintained & clean
- comments: observed opening into ceiling and water damaged ceiling tile above ventilation hood in kitchen food prep area. Must seal opening and repair or replace ceiling tile.
56. Adequate ventilation & lighting; designated areas used
- comments: observed grease build up on ventilation hood and filters above cooking equipment. Must clean and maintain.
57. All food employees have food handler training
- comments: facility missing food handler's certificates. Must provide food handlers certificates for all the required employees and maintain them on premises at all times.
58. Allergen training as required
- comments: observed missing allergen training for 1 of 3 certified food managers on site at time of inspection. Instructed manager to obtain proof of allergen training for each certified food manager on site.
