10. Adequate handwashing sinks properly supplied and accessible
- comments: observed improper maximum water temperatures ranging between 143.8-144.3f at handwashing sink in men and women's washrooms. Instructed a handwashing sink shall be equipped to provide water at a maximum temperature of 120f through a mixing valve or combination faucet. Priority foundation violation 7-38-030(c) citation issued.

38. Insects, rodents, & animals not present
- comments: observed opening at base of both front entrance doors, at base of rear exit door near basement stairs and at base of dining room rear exit door. Instructed to seal all outer openings to prevent pest entrance.

44. Utensils, equipment & linens: properly stored, dried, & handled
- comments: observed spoons, knives and forks stored laying down in uncovered container on table behind pos system and on table in dining room. Instructed to store utensils inverted or covered during storage to prevent contamination.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed rust on shelving units in walk in cooler and walk in freezer. Instructed to remove rust from all shelving units in walk in cooler and walk in freezer nonfood contact surfaces of equipment shall be constructed of a corrosion resistant, nonabsorbent and smooth material.

57. All food employees have food handler training
- comments: at time of inspection facility provided no proof of food handler training for any food handlers. Instructed to provide food handler certificates for all food handlers.

58. Allergen training as required
- comments: at time of inspection facility provided no allergen awareness training certificate for employee with city of chicago food service sanitation certificate. Instructed to provide proof of allergen training for all employees with city of chicago food service sanitation certificates.
