- comments: the pic does not have a valid chicago cfm's certificate; a copy was given. Instructed to always have the original chicago cfms certificate on the premises. Priority foundation 7-38-012 no citation issued
10. Adequate handwashing sinks properly supplied and accessible
- comments: the hand-washing sink (closest to the stalls) in the boys 2nd fl toilet room hot water does not stay on long enough to adequately wash hands. Instructed to open hot water faucet so that it stays on for at least 15 seconds. Maintain same.
41. Wiping cloths: properly used & stored
- comments: wiping cloths not properly stored when not in use. Instructed to store in sanitizing solution (the sanitizer must be at the required ppm) when not in use. Maintain same.
51. Plumbing installed; proper backflow devices
- comments: the middle faucet on the 3-compartment sink in the prep area leaks profusely from the base. Instructed to fix faucet and maintain good plumbing.
