Report date: Jun 28, 202110. Adequate handwashing sinks properly supplied and accessible
- comments: coldwater at the 1st floor cooks line hand washing sink not accessible at the faucet handle. Must turn on the cold water for use from the valve under the sink to access cold running water. Instructed to repair to provide cold and hot water from the faucet handles. Priority foundation violation 7-38-030(c) citation issued.
16. Food-contact surfaces: cleaned & sanitized
- comments: noted the interior upper water reservoir of the ice machine with black slime substance on water/ice, that is for consumption, surfaces throughout and dripping down into stored ice. Instructed to remove and discard all ice, clean and sanitize all affected areas. Priority foundation violation 7-38-005 citation issued.

- comments: all bulk spices not stored in the original containers must be labeled with the product common name.
39. Contamination prevented during food preparation, storage & display
- comments: must remove the water filter used to provide potable drinking water from the 1st floor dishwasher/bar hand washing sink. Hand sinks must be used for handwashing purposes only.

- comments: all stored items throughout the basement private dining room must be elevated from the floors.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: must not use milk crates as a means of elevation for stored items throughout.

- comments: noted a food encrusted table top can opener and wall mounted fry slicer. Must clean after each use.

- comments: cold water doesn't completely turn off on the bar three compartment sink. Must repair.

- comments: the walls in the following areas exposed, raw brick/stone, uneven with open cracks and crevices: walls in the 1st floor washroom, wall at the basement dry food storage, wall backsplash at the 1st floor hand washing sink. Instructed to seal/cover to be completely smooth and cleanable. ----wall at the basement dishwashing machine with black mold. Instructed to remove/clean and maintain. ----missing wall tiles at the basement prep area. Must replace.
55. Physical facilities installed, maintained & clean
- comments: noted cleaning tools being improperly stored inside the dish area three-compartment sink. Instructed to properly store all cleaning tools correctly, invert soiled mops at the mop sink.
60. Previous core violation corrected
- comments: previous core violations from 8-2-19 report #2304067 not corrected. #51- noted no one compartment sink at the front prep area (kitchen) on the first floor. Instructed to install a one compartment sink. #58-: noted certified food manager with no allergen training certification. Instructed management that all food service managers are required to have the allergen training certification. Priority foundation violation 7-42-090.
