Closure Date: 08/14/2019Live and dead roaches were found; there were mouse droppings, dirt, and debris on the floor throughout the facility; there was an insect control device located in food prep area with potential to contaminate food, equipment, and /or utensils; potentially hazardous ready-to-eat food, prepared in the food facility and held for more than 48 hours, located in the reach-in refrigerator, was not date marked; food employee did not follow proper handwashing procedure and use soap and/or warm water; soap was not available at the handwash sink in the food prep and restroom area; the floor tiles were cracked and severely damaged throughout the establishment; there was a waste receptacle with no lid in the unisex restroom; there was an unapproved refrigerator, reach-in freezer, and microwaves that were in use; the handwash sink was not properly sealed to the wall and the piping was in disrepair; the facility lacked a sanitizer test strips to monitor sanitizer concentration
