- comments: backflow prevention device not located on espresso or coffee machines connected to city water supply. Must provide backflow prevention device.

- comments: observed peeling paint on ceiling in dish area of the kitchen. Must removed and make smooth and easily cleanable. Observed large opening in ceiling in basement dining room. Must enclose openings to prevent pest entry.

- comments: observed previous core violation not corrected from report # 2304983, dated 8/20/19. Must correct the following previously issued core violation: #51 - must install a prep/dump sink in the coffee service area - priority foundation violation 7-42-090. No citation issued.
