- comments: observed approx 10-lbs of fresh mussels in cooler on 1st floor prep area inside large metal pan,with no shellfish tags in place with product. Pic removed and discarded food at cost approx cost $ 40. 00. Shellfish tags must stay with product. Priority foundation issued 7-38-005

- comments: observed shellfish tags maintained on site for 90 days with no date on back of label for last date used. Instructed to always provide date on the back of each tag the last date shellfish was used. Priority foundation no citation.

22. Proper cold holding temperatures
- comments: observed improper temperatures of (tcs) foods inside prep cooler on 1st floor prrep area. Approx 15-lbs of chicken brest at improper temperature of 48. 0f , approx 20-lbs of pastas at improper temperature of 46. 0f,47. 0f. Instructed to maintain cold tcs foods at 41. 0f or below. All foods removed and discarded by pic at cost approx $ 85. 00. Priority violation issued 7-38-005.

39. Contamination prevented during food preparation, storage & display
- comments: observed sewer lines not properly shielded in basement prep area above prep table. Instructed to encase sewer lines in basement prep area to intercept potential drips.
