Report date: May 15, 202538. Insects, rodents, & animals not present
- comments: observed evidence of rodent activity on the premises. Noted approximately 100 mouse droppings in various locations throughout facility (along the walls and under radiator in front space where highchairs are stored, on the floor and documents in dining room "office" behind partition, underneath sheet pan rack in kitchen near handsink, on a sheet pan on rack with roller and pizza cutters, on the floor behind "faulds oven" and a sheet pan on the floor underneath 4 door cooling unit). Instructed to remove all rodent droppings and sanitize all affected areas to monitor new activity properly. Priority foundation #7-38-020(a). Citation issued.

- comments: noted a 1/2 inch gap at the bottom left corner of the rear exit door. Must provide a tight-fitting seal at all outer openings to prevent pests ' entry.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed drink crates being used to elevate sheet pan rack in kitchen next to hand-sink. Must remove and provide proper shelving that is non-porous, smooth and easy to clean, and a minimum of 6" off the floor and provides floor accessibility to ensure adequate facility cleaning.
55. Physical facilities installed, maintained & clean
- comments: noted clutter in dining room "office" behind partition and on the floor underneath service area (next to drinks cooler). Instructed to remove all unnecessary items. All kept items should be stored on proper shelving that is non-porous, smooth, and easy to clean and a minimum of 6 inches off the ground.
55. Physical facilities installed, maintained & clean
- comments: noted hole at the base of wall and baseboards detached from walls in men's washroom. Hole in floor near door hinges of walk in cooler and hole in wall near rear exit. Instructed to repair and maintain.
58. Allergen training as required
- comments: observed no certified food manager with allergen training. Instructed any persons with the city of chicago food sanitation certificate to comply with the completion of training. Must provide documentation.
