- comments: observed hand washing sink not accessible in dishwashing area. Observed tube connected to utility line routed directly into hand washing sink. Must reroute into an approved drain. Priority foundation violation. 7-38-030(c). Citation issued.

- comments: observed black slime in ice machine dripping onto ice used for human consumption. Must effectively clean and sanitize ice machine. Priority foundation. 7-38-005. Citation issued.

- comments: no pest control log book provided during inspection. Instructed to provide a pest control log book with service reports from a licensed exterminator. Reports must be kept on file from the last 12 months along with the illinois pest control license number. Priority foundation violation. 7-38-020(c). Citation issued.

- comments: observed over 20 mice droppings throughout establishment. Mice droppings were found under hand sink in rear service station, 2nd floor storage closet and under food storage racks on 2nd floor. Must eliminate all evidence of pest activity on premises. - comments: observed female food employee in kitchen preparing food without wearing a hair restraint. Must wear an effective hair restraint when preparing food.

- comments: observed several wet wiping cloths stored on prep tables. Instructed to store in sanitizing solution bucket when not in use.

- comments: observed rice stored in garbage bucket. Instructed to use food grade containers to store food.

- comments: observed no chemical test kit on site. Must provide at all times. Priority foundation violation. 7-38-005. Citation issued.

- comments: observed heavy grease build up inside of cooking fryers. Must clean and maintain.

- comments: observed no hot water throughout establishment. Water temperature found at 85f at hand washing sink at dish washing area, 3-compartment sink, bar hand sinks, and toilet room hand washing sinks. Water temperature must be at 100f or above at hand sinks and 110f or above at 3-compartment sink. Priority violation. 7-38-030(c). Citation issued. Plumbing issue corrected on site. Water temperature is reaching 100f at all hand sinks at 110f at 3-compartment.
