- comments: observed 3 live cockroaches on the floor of the front food prep area underneath the floor mats. Additional pest control service is needed to eliminate the pest activity. Priority foundation 7-38-020(a). Citation issued.

- comments: none of the employees or food handlers were wearing a hair restraint in the food prep area. Management instructed that all food handling employees must wear a hair restraint.
